And fifth is therapy reports. These are invariably narratives of what a patient has said and done and what the therapist notes in the patient’s physical or mental condition. In physical therapy, the reports detail what exercises were given to a patient and how much of it was accomplished. It includes some progress notes and plans for further sessions. In psychiatric sessions, it’s a transcript of what transpired in a session and the therapist’s observation.
